Port forwarding to make my SIP phone work

Good afternoon. I use a SIP phone on my computer. Since with NOS at my new house the call quality is very bad. My SIP provider tell me that i need to disable SIP ALG and also disable UDP. The aminhanet.nos.pt portal does not give access to these settings :/

My SIP provider told me i need to set up port forwarding to allow certain IP ranges and also open some ports (see below). Please can some one help me please? I think i just need a screen shot example for each process on the aminhanet configuration/port forwarding page. Thank you in advance.

Instructions of what i need to set up:

Open

from 5.22.140.128  to  5.22.140.159
from 185.59.180.64 to  185.59.180.128
from 185.59.180.48 to  185.59.180.63
from 212.126.39.1  to  212.126.39.31
from 185.112.244.0 to  185.112.247.255
from 185.118.228.0 to  185.118.231.255
ports 5060 and 8060 (plus any local SIP ports)
ports 10000 to 20000 for audio
